Please complete the following steps to submit your roster for online check-in:
Prepare your official Affinity tournament photo roster.
Save a copy of your Affinity photo roster as a PDF file.
Upload your completed roster and any guest player forms here:

Rosters must be received by Monday, August 25th at midnight.
Once your roster is approved, we’ll add a watermark and notify you. You’ll need to print 6 copies of the approved roster and bring them to each game. Digital rosters will not be accepted.
🚨IMPORTANT - IF YOU ARE REGISTERING MORE THAN ONE ATHLETE: Please follow these steps to ensure each child gets the right info:
1. Fill out a separate form for each child. Each athlete needs their own registration submission.
2. Use the same parent email for all children: This ensures you’ll get all correct confirmation and follow-up messages for each of your children.
3. Use a different phone number for each child. If you do not have two phone numbers just change the last digit it a "1".
This ensures our system keeps each athlete’s information separate and sends the correct emails for their team.
Teams who do not complete the check-in process will not be eligible to play.
While not required at check-in, each team must have the following at every game:
A medical release form or UYSA Participant Registration Form with medical consent for every player, completed and signed.
A medical release form OR UYSA Participant Registration Form with medical consent for EACH player, completed and signed.
